By signing below,  I authorize Broken Arrow Veterinary Hospital to contact me via SMS(text message) to help serve me better. Broken Arrow Veterinary Hospital will send me text messages about healthcare-related items for my pet(s), including (but not limited to):

  1. Reminders for upcoming services (annual wellness checks, vaccinations, etc.) that are due
  2. Confirming scheduled appointment information
  3. Refill reminders for medication(s) due for my pet(s)
  4. Links to make payments for my pet medication(s)
  5. General communication between the practice and me about my pet(s)

By receiving SMS messages from the practice, I acknowledge that I will be opening up more channels of communication between myself and the practice to allow for the best health care for my pet(s).

While this form is an expression of consent, upon receiving the first SMS message from the practice, I understand that I will need to follow the message instructions with a proper reply to fully opt in to receiving future messages.

I acknowledge that standard text messaging/data rates may apply upon receiving SMS messages to my cellular phone and that I may receive several messages in a given period of time.

I understand I am under no obligation to authorize Broken Arrow Veterinary Hospital to send me text messages as part of receiving health care for my pet(s).

I may opt out of receiving this method of communication from Broken Arrow Veterinary Hospital at any time by:
